If the worst comes to the worst, I could probably send you a photocopy
of this manual by ordinary mail. (Always assuming, that is, that I
can find it! I did have a copy here somewhere, once.......) But -
especially if you're outside England - I'd ask for you to make a
contribution to my postage costs etc (in lieu) in the form of a
donation, directly by you, to the childrens' medical charity of your
choice.
