Does anybody make NiCd/NiMH smart chargers built into wall-warts
(wall-transformer power supplies)?

This is for an industrial application that might be big enough for a custom
order.

The first step in designing a smart charger is showing some
intelligence in reviewing the application. This normally restricts
smart chargers to specific mechanical battery pack sizes, or standard
cells.

The most common 'smart' charger using a wall-wart source will employ
the battery housing and electronics of the host target, as these
pre-exist in the ideal configuration for the application. This
functionally decouples the power source from the charging application,
allowing the use of a generic wall-wart.
