Need help to identify a part

Can anyone help with this:

It is in a T0-220 package (of sorts - 5 leads coming from the bottom vs 3) and has the following markings:

~ 8612 (there are 2 ~ one just below the first one, unable to display it in a text message - the 'humps' are a bit more pronounced).

8612 is likely a date code for the 12th week of 1986. The part could be an
audio amp from
National Semiconductor. How is it used?
 
It is used on a LCD driver board along with a Hitachi HD44790 LCD driver an a
NCR 52801 (which seems to be some sort of storage eprom). Bascially it's a digital
odometer circuit with no display and I thought I could save some money to repair it vs
buying a whole new guage package.

Pin 1 has a 12v supply to it and pin 5 has a 5v supply to it.
