anyone ever see a problem like this?

I am guessing that there is an intitial overcurrent condition during
start up(this typically happens as soon as the motor operates under
load for the first time). After I replace the output transistor
everything runs fine.

Or an inductive spike. A bit more info would help. What type of motor?
What type of driver?
