It's an antique 512x8/1024x4 bit factory programmed PMOS ROM.

It's an antique 512x8/1024x4 bit factory programmed PMOS ROM.
I scanned it and will load it to
ftp://ftp.eskimo.com/u/m/mzenier/MM5232.pdf
It's not a very informative datasheet, (also those old newsprint
databooks don't scan well), but it should be enough for you
to figure it out. There's the not very well documented Mode
pin, and the two chip enables are configurable to use either
logic level.
If you want the information in National's 1977 Memory
Applications Handbook that tell you how to format the data onto
punched cards to send to the factory, I can scan that, too. ;-)
