For BGAs?

I don't use Protel, but adding them to the footprint would seem the
right way to do it. The company doing your board assembly should be able
to advise where to place them.

I use DXP, and when I generate Gerber files, the board is displayed in
Camtastic with the fiducials placed automatically. What are you trying to
do?
