"Fabrication print" ??

Hi all. I'm planning to get a (4-layer) protoytpe board printed by one
of the several PCB prototype fab services on the web. Most of them
require a "fabrication print". As I understand it, this is a Gerber
file containing the baord outline, and symbols showing where drills of
various sizes are used. Am I correct? Is there other information that
is standard? Can someone point me towards an example?

Critical dimensions, tolerances, material, finish, copper weight,
slots, V-groove and other such information. The desired format is an
issue for the individual board houses, but Gerber seems to be popular
now.
